- Birth Certificate (NSO/PSA)
Grade I:
- must be 6 yrs. old on June
- Birth Certificate (NSO/PSA)
Grades II to VI :
- Report Card duly signed by
the Principal
- Birth Certificate (NSO/PSA)
- Good Moral Certification
SPED:
-Developmental Physician's Assessment/Evaluation
-Birth Certificate (NSO/PSA)
-Barangay Certificate
ALS:
-Report Card duly signed by
the Principal
-Birth Certificate (NSO/PSA)
